OK, I've got a server running LAMP (PHP 5.2.3, MySQL 5.0.45) and am running the vb_test script as instructed in other forums/threads. I am consistently getting fails on the MySQL support. I have created a database on our server, and an account that has been granted ALL on that database, but apparently that is not enough access for the test script.

FYI, here are the privileges that have been granted to the test account:
Global Privileges:
SELECT
CREATE TEMPORARY TABLES
Database-Specific Privileges:
SELECT, INSERT, UPDATE, DELETE, CREATE, DROP, REFERENCES, INDEX, ALTER, EXECUTE

The db user need full permissions to the database. What is the exact results you are getting?

You'll want at least 4.0.16, yes.
You need: ALL PRIVILEGES
It looks like the one that's missing is: Create Temporary Tables
If you still have a problem you will need to provide the exact results you are getting.
OK, I guess those were new priv's added with the MySQL upgrade - I have now granted ALL on that database, reloaded the priv tables, and the test script seems to be working.
FYI - I was running MySQL 3.2.x before when the database and user were created - the missing privs were not available in that older version (which is common on many hosted servers). Something to keep in mind for future inquiries...

vBulletin 3.6.X doesn't support MySQL 3.23.X. Minimum required version is 4.0.16. The unsupported version is most likely the cause of your previous problems.
